I give my approval for my child's participation in the Outlaws activities listed above. I assume all risks and hazards incidental to the conduct of the activities and the transportation to and from the activities and in the event of injury to my child, I waive all claims, and do further release, absolve and hold harmless Hit & Run Outlaws / Hit & Run Sports Center, its organizers, board members, directors, managers, coaches, supervisors and sponsors. I acknowledge that my child is in good health and can participate in athletic activities. In case of accident or injury, I understand that Outlaws will attempt to contact me first, then my emergency contact listed above, but in any event, Outlaws may contact the local police and request emergency assistance. The Hit & Run Outlaws shall not be held responsible for any injury or loss of property suffered by any individual while playing, practicing, or engaging in any activity. The participant and/or parent/court-appointed guardian of the participant acknowledges the ongoing COVID-19 virus pandemic and acknowledges the potential adverse health effects of contracting the COVID-19 virus. The participant and/or parent/court-appointed guardian agrees, acknowledges, and understands that the Hit & Run Outlaws makes no representations or warranties concerning the cleanliness of the facilities or that the facilities are free of COVID-19 virus. The participant and/or parent/court-appointed guardian further agrees, acknowledges and understands that although the Hit & Run Outlaws has sanitized and cleaned the facilities prior to the scheduled activity, the Hit & Run Outlaws makes no representations or warranties concerning the effectiveness of their cleaning with regard to the COVID-19 virus. The Hit & Run Outlaws respectfully makes the reasonable expectation/ask that participants and/or parent/court-appointed guardian be aware of the guidelines set forth by the state and federal laws applying to the COVID-19. Therefore, the Hit & Run Outlaws respectfully makes the reasonable expectation/ask the participant and/or parent/court-appointed guardian to acknowledge that it is their responsibility to comply with state/federal law guidelines and that the Hit & Run Outlaws shall not be responsible for anyone contracting COVID-19. UNDER NO CIRCUMSTANCES SHALL THE HIT & RUN OUTLAWS, ITS BOARD MEMBERS, DIRECTORS, EMPLOYEES OR VOLUNTEERS BE LIABLE TO PARTICIPANT, PARENT/COURT-APPOINTED GUARDIAN OR ANY VISITORS, INVITEES, OR OTHER INDIVIDUALS FOR ANY INJURIES OR DAMAGES INCLUDING BUT NOT LIMITED TO DIRECT, INDIRECT, CONSEQUENTIAL, SPECIAL, INCIDENTAL, PUNITIVE, EXEMPLARY OR ANY OTHER DAMAGES OF ANY KIND RELATING TO THE COVID-19 VIRUS. PARTICIPANT AND/OR PARENT/COURT-APPOINTED GUARDIAN OF HEREBY AGREES TO INDEMNIFY THE HIT & RUN OUTLAWS AND ITS BOARD MEMBERS, DIRECTORS, EMPLOYEES AND VOLUNTEERS AND SAVE THEM HARMLESS FROM ANY LOSS, DAMAGE, OR EXPENSE OF ANY KIND ARISING FROM OR ALLEGEDLY ARISING FROM ANY CLAIM, DEMAND OR LAWSUIT, OR OTHER LEGAL PROCEEDING, FROM ANY PARTICIPANT USE OF THE FACILITY OR THE COVID-19 VIRUS.
